Job Title: Social Worker

Position: Full-Time

Location: Bethany Home Sioux Falls

Starting Salary: $50,000 - $60,000 per year. Compensation is based on years of experience. 

Bethany Home Sioux Falls is seeking a dedicated and experienced Social Worker to join our team. In this key full-time role, you will work closely with our residents to identify and address their psychosocial, mental, and emotional needs, helping to provide, develop, and facilitate access to services that meet those needs. You will play a critical role in fostering an environment that promotes individuality, independence, and dignity for our residents, ensuring they achieve the highest possible level of physical, mental, and psychosocial well-being.

Key Responsibilities:

- Review facility policies and procedures as part of the interdisciplinary team to ensure compliance with state and federal regulations.

- Develop, maintain, and utilize a current list of community resources beneficial to residents and their families/significant others.

- Ensure all social service documentation meets government requirements.

- Collaborate with the interdisciplinary team and administration to protect resident rights and promote psychological well-being.

- Prevent and address resident abuse as mandated by law and professional licensure.

- Provide support, information, and organization to residents, families, significant others, and staff to encourage proactive self-advocacy and improve the quality of life and care within the nursing home and the community.

- Facilitate discharge planning.

Qualifications:

- Bachelor’s or Master’s degree in social work.

- LSW or LCSW license, or the ability to obtain licensure.

- Experience in long-term care or case management discharge planning.

- Strong work history.

- Commitment to team-oriented outcomes and care.

- Excellent communication and leadership skills.
